Registration

Register for the conference in advance by February 16, 2009 by three easy methods. After February 16, all registration will be on-site.

Register Online

Register online for the MSTA conference to pay by Visa or MasterCard. Registration deadline for online is February 16, 2009. NOTE: If you are doing the team member registration, this must be done by mail. Late fees will apply to any registration that is received after February 16, 2009.

Mail or Fax

Download the registration form to mail or fax and take advantage of the mail-in specials for team registration and first-time attendees.

By mail: Complete and return the registration form with your payment to MSTA, 3300 Washtenaw Ave., Suite 220, Ann Arbor, MI 48104. Make check or money order payable to MSTA. You may also pay with Visa or MasterCard. Purchase orders will not be accepted. To receive early registration rates, registration along with payment must be received by February 16, 2009. Late fees will apply to any registration that is received after February 16, 2009.

By Fax: Complete the registration form and fax it to the MSTA Office @ 734-973-3487, or 734-677-2407. Fax registrations can ONLY be accepted with Visa or MasterCard payment and must be received by February 16, 2009. Purchase orders will not be accepted. Faxing your registration form without payment DOES NOT register you. Payment MUST be sent with your registration form. Late fees will apply to any registration that is received after February 16, 2009.

Please NOTE: After February 16, 2009, registration must be done on-site. Please provide all requested information on each section of the registration form. Completeness, accuracy, and legibility will ensure that your registration is handled smoothly and efficiently. If you have any further questions, please call 734-973-0433.

Confirmations - Confirmations for registration will be emailed/mailed to those whose registration and payment arrives on or before February 16, 2009. Conference packets will be distributed onsite at the pre-registration desk, center concourse of the Lansing Center, and will not be mailed. Refunds and Cancellations - General request for refunds will be honored if received in writing or Fax (734-677-3287) by February 19, 2009. All refunds are subject to a $15.00 cancellation charge, and do not apply to dues. Dues are not refundable and cannot be transferred to another individual. Conference substitutions may be made on or before February 19, 2009. Person(s) substituting will need to pay dues. If a refund is needed due to unforeseen illness, or medical reason, our request will be accepted for up to 30 days after the conference (April 6, 2009).
